Sales Awards Rules & Regulations

Agent Status Declaration Form

To participate in the sales awards program and earn team credit, team leaders must complete the Agent Status Declaration Form. Any changes to a team (adding or removing agents from the team, etc.) must be indicated on this form and submitted as soon as the change occurs.
